Grant Applications are to be submitted before any expenses have been incurred for which Foundation funds are sought (and before the program has taken place).
You may fill out the applicable form by typing in your responses and saving the form. You may also print out the form and fill it in. Use “N/A” if an item is not applicable.
Grant Application Form – General – This form should be used to apply for a program to be considered during either the Fall or Spring Grant Review Cycle.
Grant Application Form – for Professional Development – This form should be used to apply for a Professional Development Program and may be submitted throughout the year.
Grant Reimbursement Request Form – General – This form should be used to request reimbursements for expenses from a Signature Project or General Grant that has already been awarded.
Grant Reimbursement Request Form – for Professional Development – Professional Development Funds Statement of Expenses: This form should have been submitted as part of the Professional Development Application with the estimated expenses of the program, and must be submitted again within 30 days of the completion of the program with the actual expenses in order to request the reimbursement.
